During the Renewal Inspection on 10/27/2025, the wooden picnic table on the outdoor playground was cracked and had sharp edges exposed. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. During the Renewal Inspection on 10/22/2025, the most recent health report on file for Child #1, a young toddler was dated 1/28/2025. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. On 10/22/2025, the updated health assessment on file for Staff #1 dated 9/27/2024 did not include an examination for communicable diseases and the results of that examination. The health assessment on file also did not include the physician's or CRNP's assessment of the person's suitability to provide childcare. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. The DHS health assessment form will be brought to this appointment to make sure that the doctor specifies that the staff person is free of communicable diseases and suitable to provide childcare. On 6/5/25, Staff #1 had a volunteer Pennsylvania child abuse clearance on file, which is not suitable for employment purposes. Staff #1 was observed to have direct contact with children during the inspection. See code sheet for date of hire. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). All required staff clearances must be for employment. A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1 may not work in a childcare position at the facility until the Pennsylvania child abuse clearance for employment purposes is on file. During an unannounced inspection on 6/5/2025, peeling paint was observed on the wall in the front area of the room located on the left side of the building upon entering the facility. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Painted where needed. On 10/23/2024, Staff #1 it was discovered that Staff #1 has resided outside of the state of Pennsylvania during the previous 5 years. Staff #1 did not apply for the out of state clearances prior to employment as required by the CPSL. Staff #2 had current clearances on file on 10/29/2024. However, the updated state police clearance on file for Staff #2 was dated 8/6/2024. The previous state police clearance on file for Staff #2 was dated 4/23/2019. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). During the Renewal Inspection on 10/23/2024 and on 11/5/2024, the facility did not have a qualified director present at the facility site for a minimum of 30 hours per week. Requirement: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week. Correction: We have a staff person who recently left returning to work that is qualified to be the director. This individual will be the full-time director beginning next week. Inspection: Renewal.
