Facility staff # 1 did not complete PQAS approved pediatric first aid and cpr by the date of expiration on their current certificate of 4/26/25. Requirement: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Fire drills were not conducted in 60 days from 7/22/24 to 9/22/24 (62 days). Requirement: The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met. The facility alarms were not tested in 30 days from 7/22/24 to 8/22/24 (31 days). Requirement: The operator or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Child # 1,2,3, & 4 do not have copies or previous copies of the original or subsequent fee agreements forms or emergency contact forms since time of enrollment. Copies on file are more than 6 months old from time of enrollment and they are the only copy the operator saved. Operator reported throwing out all previous emergency contact forms and fee agreements for all 4 children since the time of opening. Requirement: A child's record must contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement. Correction: I will make sure to keep child files and all paperwork from time of enrollment until at least 1 year after a child withdrawals from the facility. Child # 1,2,3 & 4 enrolled and attending the facility more than 6 months do not have a child service report completed. Requirement: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e). Clean, age appropriate (3290.105(a))

Rest equipment is not labeled with the assigned child's name who uses the equipment on the providers cot number list. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Correction: The equipments were label at the time of inspection.
