Observed a child's bottle of milk on a cot, and was not refrigerated for a period of approximately one hour. Small Toys and Objects (3270.103)

Observed plastic bags accessible to the toddlers in care due to a failed child safety lock, in a low cabinet below the counter in the toddler room. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Building Surfaces (3270.76)

Observed ceiling vents in the toddler and preschool room which needed to be cleaned. Observed a frayed carpet under the toddler changing table. Observed a carpet runner in the toddler room which had rolled up edges creating a tripping hazard. Observed 3 exposed bolts on and beside the toddler gate of the outdoor play area. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The ceiling vents were cleaned. The frayed carpet under the changing table was replaced and the carpet runner in the toddler room was thrown out. The bolts on the fence were covered. Infant sleep position (3270.119)

Observed two infants sleeping on inclined bouncy chairs which had been placed in their assigned cribs. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. Correction: Moving forward, if a child falls asleep in a bouncer, they will be moved to their cribs and placed in the AAP recommended sleeping positions. Inspection: Unannounced Monitoring. Observed the emergency contact information in the child care space did not contain all of the emergency contact information required to be present in the child care space. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Information re: medical problems (3270.151(c)(4))

Observed the updated health assessment for staff file #1, which was filled out to say that the staff was not suitable for child care but did not contain any clarifying information. Requirement: An adult health assessment must include information on medical problems that might threaten the health of the children or prohibit a staff person from providing adequate care to children. Observed child file #2 did not contain complete documentation of the polio immunization in accordance with the schedule recommended by the ACIP. Requirement: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Correction: Fix the area where the screw had gone through and securely anchor the shelf to the wall. Replace the broken tote with a new one. Inspection: Renewal. Observed the preschool first-aid kit was accessible to children while hanging on a doorknob in the outdoor play area. Requirement: A first-aid kit must be inaccessible to children. Observed hand sanitizer, labeled "keep out of the reach of children", accessible to children on a counter in the preschool room. On 6/18/25 observed the door to the toddler bathroom and the preschool bathroom does not shut completely creating a pinch point and also exposed drywall by the preschool toilet. Correction: Took the arms off of doors so they can close as needed to prevent pinch points. Repaired drywall next to toilet. Inspection: Initial review. On 6/18/25 observed the diaper changing table in the infant room rolls and was not stable. Observed a toy shelf in the preschool room which had an exposed metal hinge. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Took the wheels off the changing table to ensure it was stable and would not roll. Also, removed the hinges off of the shelf. Inspection: Initial review.
