During the full renewal inspection, it was observed that the lunch boxes for the school-aged/PreK children contained items of potentially hazardous food types and were left unrefrigerated upon child arrival until the serving of a morning snack time. During the full renewal inspection, it was observed that the outdoor play space used for the young toddlers had multiple small pieces of mulch less than 1 inch in diameter present and child accessible on two inspection visits. There is no applications in the file for an updated FBI clearance or a PA criminal clearance. Requirement: Staff person #1 must be removed from a childcare position by the close of business because she/he does not have all the required clearances or requests for clearances [Effective 3.13.25]. Correction: The file of staff person #1 contains a FBI clearance dated 1.13.20 which is more than 60 months old AND an PA criminal clearance dated 1.10.20 which is more than 60 months old. There are no applications in the file for an updated FBI clearance or a PA criminal clearance. This violation occurred because staff person #1 did not complete all the necessary requirements. This was not caught by the director because he neglected to follow up with staff person #1 to see if she completed it. If the required clearances are not obtained within 45-days, the employer must dismiss the employee or move them to a position where they do not have "direct contact with children". If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. The child care staff who have not completed health and safety training must be supervised by a trained, qualified child care staff person until untrained staff have obtained training. Correction: I took our program's use of pedestrian and vehicular routes to our local and state police to obtain signatures and deliver a copy of our plan to them. Inspection: Renewal. Upon inspection of each of the childcare spaces, it was found that the child emergency contact information sheets for multiple children were out of date and did not match master file updated information in the following spaces: Young Toddler, Pre-K 1 and Pre-K 2. Requirement: If emergency information is updated in a master file, it shall be updated accordingly in other facility records. Upon inspection of each of the childcare spaces in the facility, it was found that a written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not posted in any of the spaces. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every childcare space and shall accompany a staff person who leaves on an excursion with children. Upon inspection of the children's records, it was found that the developmental reports for Child #2 were dated 8.8.23/5.8.24 [9 months]; for Child #3 8.1.23/5.8.24 [9 months]; and for Child #4 8.1.23/5.8.24 [9 months]. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Outdoor equip.- protective surfacing (3270.102(c))

Upon inspection of the outdoor playground equipment, it was determined that the mulch surrounding the large climbing equipment was 1"- 2" in depth. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: I installed playground boards around the playground equipment in question and filled area over 9 inches deep with playground mulch. Inspection: Renewal. Upon inspection of each of the childcare spaces in the facility, it was found that ceiling tiles were damaged in the following spaces: School aged [1 broken] and Pre- K 2 [water damaged]. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The actual leak in the roof was fixed well before the inspection. The ceiling tiles were updated to get rid of the water stain from the leak. Inspection: Renewal. Correction: We notified the parents of their mistake in leaving toxic materials in their child's cubby and sent the product home with them. Inspection: Renewal. Upon inspection of the facility emergency plan and the required letter to parents, it was found that the letter to the parents was missing information related to lock-down procedures, methods of communication and any applicable accommodations.
