The file for staff persons #2 contained only 4 of the required 12 hours of child care training during the previous 12-month period of 12/2023 to 12/2024. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: The staff member had completed the number of trainings needed already to be in compliance. Thus correcting any of the missing hours from the year 12/2023 to 12/2024 . Inspection: Renewal. Small Toys and Objects (3270.103)

Observed a Styrofoam plate for a craft project accessible to children still placing objects in their mouths in the T1 classroom. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Inaccessible to children (3270.75(b))

Observed the backpack containing the first-aid kit to be laying on the floor in the classroom accessible to the children in care. Requirement: A first-aid kit must be inaccessible to children. Correction: The teacher immediately picked up the bag and placed it on a high cabinet in the school age room, out of the school age children's reach. Inspection: Renewal. Child #1, an infant, swallowed a push pin, which was not seen by Staff #1, and was taken to the Emergency Room. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Bulletin boards were taken down in the infant room and both toddler rooms. This taking all push pins out of the rooms. This was done ASAP, the same day of the incident. Another way was added to the bulletin boards that was safer for the children for the teachers to hang up DHS and facility paperwork. All teachers were reminded of the less than 1 inch regulation, and another safety checklist was completed by the staff during nap time. Inspection: Incident Investigation. General Health and Safety (3270.21)

Child #1, an infant, swallowed a push pin, which was not seen by Staff #1, and was taken to the Emergency Room. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The incident was a true accident that the staff member and the organization are deeply sorry for. The children's safety has always been a priority and will always be. Swift action was taken immediately to make sure the child was safe (she was completely fine) but the staff in attendance went above and beyond for the child and the family is very grateful. The infant room, the bulletin board was removed ASAP and replaced with something that would not require any push pins. The room was tripled check to make sure there was no other items that were less than 1 inch in the classroom. The toddler rooms as well, were checked and all bulletin boards were removed. Supervised at all times (3270.113(a))

Child #1, an infant, swallowed a push pin, which was not seen by Staff #1, and was taken to the Emergency Room. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Staff was reminded that staff paperwork can be done in the morning when the children are not present. That you must at all times be facing the children, you should not have your back towards a child. This would of possibly prevented the incident.
