At time of renewal inspection on 10/14/25, the material covering the foam wedges/furniture in the young toddler room was worn thin and in need of being replaced. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Refrigerator (3270.107)

At time of renewal inspection on 10/14/25, the refrigerator in the Pre-K classroom did not have a thermometer inside to ensure the temperature remains at 45 degrees Fahrenheit or below. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: During the inspection a new thermometer was placed inside the refrigerator. Inspection: Renewal. Building Surfaces (3270.76)

At time of renewal inspection on 10/9/24, the ceiling vents in the PS/PK rooms were visibly dusty. Two ceiling tiles in the room were also observed to be stained. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The ceiling vents in the rooms will be cleaned so there is no visible dust or dirt. The ceiling tiles will be replaced so there are no stains . Inspection: Renewal. No peeling paint or plaster (3270.77(a))

At time of renewal inspection on 10/9/24, chipped white paint was observed on upstairs bathroom door, and on the blue and yellow trim around the door between the infant and young toddler room. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. At time of renewal inspection on 10/9/24, the emergency information for child #1 did not include the phone number for the child's physician. At time of renewal inspection on 10/9/24, the health assessment for staff person #1 states that a physical examination was not performed and question #3 regarding the individual's suitability to provide child care was not answered by the physician. Requirement: An adult health assessment must include a physical examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care. Correction: Staff person #1 will be notified to return to the physician so a physical can be performed and question #3 can be answered. Staff person #1 will return a complete health form.
