Complaint investigation determined that Child #1 sustained a head injury at approximately 11:50 AM. The child's parent was not notified of the injury until approximately 5:30 PM when the child was picked up from care. Child #1 was subsequently taken to be medically evaluated by the parent for the injury. Requirement: If emergency medical care is needed for a child, the parent shall be contacted as soon as practical in the best interest of the child. If the parent cannot be reached, the operator shall record in writing the reason emergency care was required and the attempts made to inform the parent. Correction: Child #1 bumped head. The child's parent was in surgery with another child. At no point was there a sign/symptom of an emergency. Child was cleared upon parents arrival by medical professionals. Center will notify in writing and phone call of any/all injuries when emergency response is needed and in writing only when a minor incident occurs. Inspection: Complaints- Legal Location. Health and Safety Training (3270.31(f))
An unannounced monitoring inspection was conducted on 1/6/2026. At that time, files for all staff present during the inspection were reviewed. Staff Person #1 had documentation of American Heart Association (AHA) Adult First Aid/CPR, which is not an approved pediatric first aid/CPR curriculum. Staff Person #1 did not complete the required pediatric first aid/CPR training from a PQAS-approved instructor within 90 days of their date of hire (see LIS code sheet). Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Correction: Staff #1 was approved during recent inspection 9/2025. During most recent visit it was then denied. Staff #1 is scheduled for recertification on 2/10/26. Until then staff #1 will not be unsupervised with children and will be in ratio with other staff present. Inspection: Unannounced Monitoring. An unannounced monitoring inspection was conducted on 1/6/2026. At that time, diaper-changing surfaces were observed to have cloth coverings. The director reported that each child has an individual cloth covering, which is stored in the child's bin and cleaned at the end of the day. The cloth coverings are not cleaned and sanitized after each use. Requirement: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering. Correction: Removed cover from infant changing tables. Again, this practiced passed inspection 3x by 2 different reps. Inspection: Unannounced Monitoring.
