At the time of inspection, the mulch surrounding the embedded equipment on the preschool playground measured at less than 9 inches in depth. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: Fresh mulch was placed on the playground area, and embedded under the equipment. Inspection: Allocated Unannounced Monitoring. It was observed in the infant classroom that one bucket seat for the infant table was cracked creating rough edges, sharp corners and pinch and crush points. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Locked or inaccessible (3270.66(a))

It was observed in the infant classroom, Bleach and additional cleaning sprays were stored under the sink that was not locked due to a broken child safety lock and were accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The broken child lock was replaced with a new lock, that was then placed across the outside of the cabinet doors. Inspection: Allocated Unannounced Monitoring. Age and Training (3270.31(e)(4)(i))

Staff person #6's documentation showed that their pediatric first aid and CPR was not renewed before the expiration. (certificates were date 5/30/23 and 6/4/25) Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Fire safety - 1 yr. (3270.31(e)(4)(ii))

Staff person #5 had documentation of fire safety training 6/6/24 and an update on 8/28/25. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Staff person #4 had NSORs dated 2/24/20 and 7/1/25, child abuse clearances dated 6/17/20 and 7/3/25, and mandated reporter training dated 7/29/20 and 8/23//25 on file. On the preschool playground the Little Tykes play structure had an excessive build-up of dirt. Documentation showed that child #2's initial health assessment was dated 12/26/24 and their updated health assessment was dated 7/7/25. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Observed Pack N Plays & compact cribs in the infant classroom with full size crib sheets on the mattresses in lieu of sheets intended for use on a Pack N Play and a compact crib mattress. The sheets, which were too big for the mattress, created access material which creates a potential suffocation hazard. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. Correction: All crib sheets will fit firmly around each mattress to prevent potential suffocation hazard. Conditions at the facility will not pose a trheat to the health or safety of the children. Staff assigned to specific children (3270.113(a)(1))

Observed in the older toddler classroom: Staff person #1 and #2 were supervising a group of 13 older toddlers. When asked to identify their groups, staff person #1 named 5 children and staff person #2 named 3 children. 5 of the older toddlers were not identified. Observed in the infant classroom: Staff person #3, #4, #5 and #6 were supervising a group of 11 infants. When asked to identify their groups, staff person #3 stated she was a floater and did not have a group, staff person #4 named 3 infants, staff person #5 named 3 infants and staff person #6 names 3 infants. 2 infants were not identified in an assigned group. It was not communicated to staff person #3 who she was responsible for when she entered the classroom. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group.
