The fire safety training for staff #2 and #3 is dated 4/2/24, which is more than 12 months ago. The exit from the Rainbow Room to the lobby was obstructed by a Pack and Play. Upon interview with the operator, it was determined that the dismissal policies are not discussed with the enrolling parents. Mixed Age Level (3270.52)

At approximately 10:42AM it was observed that Staff person #1 was with a mixed group of 7 children (3 young toddler, 2 older toddlers, and 1 preschooler) in the main childcare space, while staff person #2 was heating up lunches in the office area. Leaving Staff Person #1 with a ratio of 1:7. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Correction: The microwave was moved to the room to prevent issues with ratios, and we have since been actively hiring to ensure ratios maintain. Inspection: Unannounced Monitoring.
