   During the renewal inspection cert rep observed middle room right wall with chipped paint and a hole in the wall, entry way leading to kitchen was observed with chip paint. Radiator cover in the front room was observed broken with a rusted nail. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Correction: Provider will ensure all surfaces of childcare space are kept clean, in good repair and free from visible hazards. Inspection: Renewal.
