At approximately 9:35AM, staff #1 came to the door of the facility with 4 school age children, leaving staff #2 in the Rainbow room with 8 children which included 2 older toddlers and 6 preschoolers. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b). Correction: We will make sure that we stay in child staff ratio when answering the door if we don't have enough staff in the room. Cigarettes were observed to be accessible to children in a purse observed on the floor of the Green Room. Two cans of spray disinfectant were accessible to children on the top of the cubbies in the entry hall. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. The metal floor air vent in the Rainbow Room was bent in multiple places resulting in exposed sharp areas. An empty plastic grocery bag was observed to be accessible to children in a cubby in the entry hall. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Children's Tylenol observed on a table in the green room was not labeled with the name of the child for whom it was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: We plan to paint the ceiling tiles with kills paint to seal the stains on the ceiling tiles the broken ceiling tile light is replaced, and the baseboard is fixed and secured against the wall Inspection: Renewal. Cots in use by children during nap lacked 2 feet of space on 3 sides. In each case, an empty cot between the children obstructed the space. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: Our staff member did have a health form done in the 24 month timeframe however [they] did not get the health form back and when [they] went to the doctors and request requested for that doctor was no longer there so they would not provider with the health form as soon as we found out that information, [they] immediately had another health form done so it would bring [them] current and a new health form is on file Inspection: Renewal.
