A gap was observed under the school age room door due to a missing door sweep. Three stained ceiling tiles were observed in the YT room. One stained ceiling tile was observed in the teacher's break room. The front door of the facility did not latch causing the door to remain open. Based on statements made by staff, on multiple occasions, the rear exit door in the infant room leaked causing the carpet to become wet. Children's clothing became wet after crawling or sitting on the wet carpet. Based on statements made by staff, drywall in the infant room near the exit door had visible water damage for a period of time before repairs were made. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: A new door seep was installed on the school age room door. The stained ceiling tiles in the Y.T. room and the teacher break room were painted with a sealant.
