During the complaint investigation on 6/5/26, it was verified that staff person #1, left child#1 in the Firefly classroom, alone for 30 minutes on 5/18/26 from 1:29pm to 1:59pm. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. During the renewal inspection on 8/8/25, in the Caterpillars Room, under the mattress on the changing station there was excess grime/filth accumulated on the surface. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The surface under the changing mat was cleaned using soap, water and bleach solution in the Caterpillars room. Inspection: Renewal. During the renewal inspection on 8/8/25, in the Ladybugs Room the red leather couch had torn material and was in poor repair. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The torn chairs were removed from the classroom. We are planning to purchase new chairs and replace them. Inspection: Renewal. During the renewal inspection on 8/8/25, In the Jitterbugs room, the thermometer in the refrigerator was 50° F. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: We will ensure that the refrigerator door is properly closed after opening it. The teacher will monitor the temperature using the fridge thermometer. A child lock was placed on the door to ensure it is closed correctly. Inspection: Renewal. During the renewal inspection on 8/8/25, in the Butterflies 2 Room, there was no emergency contact information in the childcare space where the children were receiving care. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: Two copies of each child's emergency contact form have been made for all children. there will be two bags red first aid bags with the copies of the emergency contact's form. During the complaint investigation on 7/24/25, it was verified through interviews and video footage that staff person #1 aggressively grabbed a child and pushed the child onto the couch, then pushed the child down a second time when the child tried to get up. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: A discussion was had with the teacher about proper handling of student behavior and it was stressed that her handling of the situation could be better and that what was done was not acceptable. The teacher was given an official written warning and she also completed the suggested Big Emotions and Self-Regulation for Adults and Children training through Better Kid Care on July 28, 2025. During the complaint investigation on 7/24/25, it was verified through interviews that staff person # 1 yelled in the face of a child. Requirement: A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: It has been stressed to the teacher to speak to children in a soft calm tone and the teacher completed the suggested Big Emotions and Self Regulation for Adults and Children training on Better Kid Care on July 28, 2025.
