In the back room on the shelf that contained art supplies was shaving cream that stated keep out of reach of children accessible to children. In the back room in the unlocked cabinet under the sink was Misty Citrus All Purpose Cleaner that stated keep out of reach of children accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Child #1's file did not contain signed parental consent for administration of medications or special dietary needs. Requirement: A child's record shall contain signed parental consent for administration of medications or special dietary needs. Handwashing signs (3270.82(h))

The bathroom in the main room was missing a handwashing sign by the toilet. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: A hand washing sign was placed above the toilet immediately. Going forward all postings will be displayed in an open space and noticeable. Inspection: Renewal.
