On August 27, 2025, during the renewal inspection, the health assessment reviewed child #3 did not contain the immunization record. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. On August 27, 2025, during the renewal inspection, the emergency contact form reviewed for child # 1 did not contain the health insurance policy number. Requirement: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. On August 27, 2025, during the renewal inspection, the emergency contact form reviewed for child #1 and child #2 did not contain the enrolling parents work address and telephone number. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. On August 27, 2025, during the renewal inspection the record reviewed for staff person #1 contained an DHS FBI fingerprint clearance dated 2/26/2020, which is more than 60 months old. (See code sheet for hire date) Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 1 may not work in a childcare position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the DHS FBI fingerprint clearance, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a childcare position at the facility. Correction: Provider will remove staff person from their position. Inspection: Renewal. During the unannounced allocated inspection cert rep observed the 7 emergency contacts were not present in the childcare space while children were receiving care. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Supervised at all times (3270.113(a))

During the unannounced allocated inspection cert rep observed staff #2 was on a cell phone in the toddler classroom during nap time. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Review 3270.113 with all staff. Inspection: Allocated Unannounced Monitoring. 110º F or less (3270.69(b))

During the follow-up unannounced allocated inspection from previous inspection dated 9/5/2024 observed the water temperature exceeded 110 degrees in the toddler room sink. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: Water was turned down again considering there was temperature variance between provider's and rep's thermometer. Inspection: Allocated Unannounced Monitoring.
