Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Correction: Have the parents get a physical from the doctor or a note stating that the child is free of communicable diseases and can participate in childcare. Note: child #1 is done at JDFC. The cots used as rest equipment by the children in the Turtle classroom were not individually labeled. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: Have the teachers number the cots in the classroom and have a list of who gets which cot and where they get placed in the classroom. Inspection: Renewal. Clean and good repair (3270.102(a))

Observed in the Elephant classroom the plastic lid to a container of Legos was cracked with a potential pinch point. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The lid has been taped up to prevent pinching. If the lid cracks any further the lid will be discarded to prevent any injuries. Inspection: Renewal. Building Surfaces (3270.76)

Observed rust on the heat registers in the Giraffe classroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The maintenance department painted the heat registers in the classroom to ensure no rust was present on the surface of the heater. Inspection: Renewal. Above ground window opening restricted to 6" (3270.72(d))

Observed the window in the upstairs bathroom used by children was lacking a restriction preventing it from opening greater than 6 inches. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: We put a sign on the window stating Do Not Open Window. The lever to open the window has been taped shut. Inspection: Renewal. Observed in a storage closet in the Giraffe classroom, Lysol spray stored next to Ritz crackers and GooGone spray stored next to a box of assorted snacks. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The toxic materials were moved to a different closet that does not contain any food items. Promote development (3270.111(c))

Based on staff interviews it is verified that child #1 has been removed from the classroom due to disruptive and aggressive behaviors and taken to the director's office which is not certified child care space and does not provide an environment that promotes the development of skills, social competence and self-esteem Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: Going forward if a child is removed from the classroom or disruptive or aggressive behaviors they will be taking by an available teacher to a certified child care space. While they are in the space to calm down the teacher will practice calming activities with the child, read a story to them, sing a song with them, give them a few options of sensory toys to play with, and/or talk to them about better choices to deal with their feelings. Based on staff interviews it is verified that child #1 has been removed from the classroom due to disruptive and aggressive behaviors and taken to the director's office which is not certified child care space. Requirement: A facility shall provide indoor child care space for individual and group small muscle activity. Correction: Going forward if a child needs to be removed from the classroom for disruptive or aggressive behaviors the available staff will take the child to a certified child care space. Observed on the outside play space a bin of assorted toys with a yellow plastic lid that was cracked posing a pinch hazard. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. General Health and Safety (3270.21)

At the time of the inspection the door to the storage room located between the Gym and the Minions classroom was open allowing children access to a paper cutter which was sitting inside the door on a low table. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: I plan on correcting this by hanging a sign on the door that states "Please keep door closed" I will let all staff members know of the safety concerns and let them know that the door needs to remained closed at all times. The fire alarm test log identified that alarm tests were conducted 1/24/25 and 2/24/25 which exceeded 30 days between tests. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Observed in the Turtles Room located on the second level, a sliding window with a modification that was not functioning properly and allowed the window to be open greater than 6 inches. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: Materials have been cut to length to allow an opening of no more than 6 inches. These materials have been placed in all windows. The materials that were too short have been removed. Observed in the Elephants Room Zyrtec and an Epipen that were not labeled with the name of the child for whom they were intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: The bag that contains the Epipen and the Zyrtec has been labeled with initials.
