During the Renewal Inspection on 6-23-26, it was discovered that thermometer in the Toddler 1 Room was not present in the refrigerator. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: Thermometer was placed in the Toddler 1 refrigerator. Inspection: Renewal. On 2-13-26 it was discovered that Staff#1 inappropriately disciplined children by mimicking biting a child who bit another child by placing their forearm in her mouth, and by pulling a child from their cot. This also resulted with the child bumping their head. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Mixed Age Level (3270.52)

In the Toddler room, 11 older and younger toddlers were observed to be in care. One staff listed 5 young toddlers in their primary care group. The second staff listed 6 children in the primary care group that contained a mix of older and younger toddlers. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). In the Toddler 1 room, the gray, play kitchen set was observed to be dirty/dusty. In the PreK room, a helicopter bench was observed to be dirty/dusty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. There was no posting indicating pick up and drop off procedures for the facility. Requirement: Safe pedestrian crossways, pick-up and drop-off points and bike routes shall be appropriately determined in the vicinity of the facility and communicated to the children and parents in writing. Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility. In the toddler 1 room a staff was observed to change a child's diaper and the staff did not wash their own hands after completing the diaper change. Requirement: A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child. In the Toddler 1 room, cobwebs and dust were observed on the window sills. In the Toddler 2 room window blinds and window sills were observed to be dirty/dusty. In the Toddler 3/Young Preschool room, window sills were observed to be dirty/dusty. In the PreK room, window sills were observed to be dusty/dirty. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Childcare Director had teachers clean window sills to remove any dust or cobwebs. The blinds in the toddler 2 room will be removed and replaced with curtains. Inspection: Renewal. In the toddler 2 room, hand sanitizer was observed to be sitting on a desk, accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Staff #1 had a child abuse clearance dated 3/5/20 and updated on 3/7/25, which is greater than 60 months. Clearance is currently up to date and on file. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: Childcare Director will review staff files to ensure that all all clearances are up to date. Staff 2 and Staff 3 did not have a letter certifying that the staff persons have completed the required classroom training. Requirement: An individual 16 years of age or older who is enrolled in an approved training curriculum may be used as a staff person if a representative of the training institution certifies in writing that the individual has completed the required classroom training and is currently enrolled in the curriculum. Correction: Childcare Director will get a letter from the CTE teacher stating that the staff have completed 2 years of training that is required in the Childcare Develpoment class at Keystone Central School District. In the PreK room, a push pin/thumb tack was observed to be holding up a purple poster on the wall that was accessible to children in the class. During a visit, the outstanding inspection summary was observed to be missing in a conspicuous area. Upon discussion with the director, director reported that the outstanding inspection summary was not posted. Requirement: The operator shall post a copy of each inspection summary issued by the Department in a conspicuous location used by parents. The inspection summary shall remain posted until an agent of the Department verifies that each regulatory noncompliance item cited on the inspection summary has been corrected. Staff #1 and #2 did a headcount in the class before leaving for an excursion. After the headcount, a child hid from the teachers as the class exited the room. The child was left unsupervised for approximately 5 minutes. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: 1. Self reported 2. in the PreK counts room, staff #1 left a child unsupervised, sleeping, for approximately 1 hour. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. 3. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of seven days after DHS approves the documentation format. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Upon notification, the Director interviewed staff who observed the incident and self-reported to DHS. She then spoke with her supervisor, the Director of Childcare Quality and Development, and RVR's HR Director in regards to the incident. Staff #1 was heard yelling and screaming at a classroom of older toddlers for approximately 15 minutes. Requirement: A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: Staff person involved was immediately released early on 10/4/2024 following incident, with pending suspension of employment until a decision of how to best move forward was made.
