Based on staff statements, Child #1 was left to cry for approximately 45 minutes while in the care of Staff #1. Child #1 was not provided stimulation by being held, rocked, talked to, played with, or carried. Requirement: Infants and toddlers shall be provided stimulation by being held, rocked, talked to, played with and carried. Correction: Staff #1 was provided with additional training and options if providing stimulation by being held, rocked, talked to, played with, or carried does not comfort a child. The file for Staff #3 contained a Child Abuse Clearance dated 5.12.2020 which is more than 60months old. The file for Staff #4 contained an NSOR clearance that was dated 12.19.24 which is more than 45 days after the staff's start date. Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The file for Staff #3 contained an initial TB screening that was conducted more than 12 months prior to the staff's start date at the facility and did not indicate the results of the screening. Requirement: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. The toilets in the boys restroom in the Preschool room were lacking handwashing signs above the toilets. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. In the infant room the child lock on the cupboard under the sink was broken making liquid tide accessible to the children in the room. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. The file for Staff #1 was lacking proof of a high school diploma. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. Fire drill logs at the facility indicate that fire drills were conducted on 12/19/24, then again on 2/26/24, more than 60 days apart. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Correction: The director will add a follow up during monthly staff meetings to ensure that staff remember to conduct a fire drill at least every 60 days. Inspection: Unannounced Monitoring. The last date listed on the documentation of the facility's smoke detector testing indicates that smoke detectors were tested on 12/31/25. Subsequent documentation of testing does not provide the date, only the calendar month and staff initials. Act 62 require smoke detectors to be tested, and testing documented, at least every 30 days. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Staff assigned to specific children (3270.113(a)(1))

At approximately 1:25 PM Facility Persons #1, #2 and #3 were supervising a total of 12 preschoolers in the preschool space. When asked to identify which children they were each responsible for supervising, Facility Person #1 indicated they were not assigned specific children for supervision purposes. Facility Person #2 claimed responsibility for five specific children, and then Facility Person #3 claimed responsibility for six specific children, including two children claimed by Facility Person #2. Two children were identified by both staff, and three other children were not identified by any staff as their responsibility to supervise. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Based on interviews, there were multiple occasions where staff #1 raised her voice in a harsh tone towards the children.
