On August 19, 2025, during the renewal inspection, the handwashing sign posted over the sink in the kitchen area did not contain the language (who and when to wash hands) The back bathroom in the childcare space did not contain handwashing signs over the sink and toilet. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: Post hand washing signs over each sink and toilet. During a renewal inspection conducted on 10/04/24, Certification Representative conducted on walkthrough of the facility and observed a small roach crawling on the infant's changing table, and another roach crawling on the wall above the changing table. Requirement: Evidence of infestation of insects or rodents in the facility is not permitted. Correction: The Owner will schedule an increase of extermination from monthly to bi-weekly until the infestation is resolved. Inspection: Renewal.
