    Observed 8/13/2025 staff person #1 was observed supervising 5 children alone with the youngest age (10 months) and oldest (5 years old) exceeding the of ratio of 1:4 . Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). The required staff: child ratios must be maintained at all times Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Correction: Staff #1 had another training on children's ratios, Director posted different examples in each classroom. Inspection: Unannounced Monitoring.     Observed 7/22/2025 Staff was observed letting child #2 continuous be in a bouncer chair during the duration of the inspection not allowing the child to have movement and confined to a space. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: To find more ways to help child fall asleep such as more play time, feeding before nap time, dim lights and soothing activities. Inspection: Renewal.
