The facility was observed to have two blue spritz bottles that contained a clear liquid but lacked a label identifying the contents. One bottle was observed in the One's Room, and the other bottle was observed in the Three's Room. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Building Surfaces (3270.76)

The facility was observed to have a hole in the wall plaster behind the door in the Three's Room. The radiator in the Two's Room was observed to have a broken/loose cover exposing sharp edges. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. The facility was observed to lack an operating thermometer in the refrigerator in the One's Room. An infant was observed sleeping with a WubbaNub pacifier, with the soft plush animal toy still attached to the pacifier and therefore not compliant with the sleeping position recommended by the American Academy of Pediatrics.
