The file for child #1 was observed to lack an initial health report dated no later than 60 days following the first day of attendance at the facility. This was evidenced by an initial health report present in the file dated 02/12/2024 which is 70 days from date of enrollment. Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The facility was observed to have the medication Albuterol labeled as intended for child #2, however, the facility lacked written consent from the parent for administration of the medication (Med Log). Requirement: A parent shall provide written consent for administration of medication or a special diet. Correction: Have the parent complete a med log at the time of drop off the next morning. The facility was observed to have a container of Benadryl that lacked the name of the child for whom the medication is intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Write the child's initials on the bottle of medication. Inspection: Renewal. Immunization record/Dismissal policy (3270.131(d)(5)/3270.131(e)(3))

The file for child #1 was observed to lack a review of the child's immunized status according to recommendations of the ACIP. Specifically, the log on file documents the child received 3 DTAP/DTP/TD and 3 Pneumococcal immunizations the ACIP schedule recommends 4 of each DTAP and Pneumococcal. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Handwashing signs (3270.82(h))

The facility's restroom by the washer and dryer was observed to lack a handwashing sign. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: We will add two handwashing signs to the area around the toilet in addition to the two signs at the sink. Inspection: Renewal. Room C in the facility was observed to have a clear spray bottle in the first aid kit that contained a clear liquid but did not have a label identifying the contents. Room D was observed to have a clear green tinted spray bottle with a liquid inside but lacked a label identifying the contents. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: We have labeled all clear spray bottles in the first aid kits stating what liquid is in them.
