At the time of the investigation, it was determined that staff #1 grabbed child #1 by the arms, yelled at child, then pushed child #1 causing child to cry. Requirement: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: All staff will take Better Kid Care trainings, "Big Emotions and Self Regulation for Adults and Children" and "Supervision: Teamwork" by 4/6/26. Inspection: Self-Reported Non Compliance. On 2/5/26 an unannounced inspection was conducted at the facility. Staff #1 was working alone in Room 8. Staff #1 left her classroom unattended for a brief period of time during a personal medical scenario where staff needed access to a bathroom to vomit. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. 2/5/26 The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. 3/24/26 The legal entity will provide policy and or procedure for staff in regards to personal medial situations while in the classroom. 3/24/26 Correction: All staff will complete Better Kid Care training "Adverse Childhood Experiences: Building Resilience" by 3/6/26. At the time of the unannounced inspection, it was found that child #1 was left in a classroom while the rest of the class were at the playground. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. 7/28/25 The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. 9/19/25 Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
