Staff #1 training records did not reflect completion of the required 12 hours of annual training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.    During a renewal inspection conducted on 036/25/25, the Certification Representative observed Staff #2 changing a child's diaper without required handwashing procedures for both staff and child as outline in the diapering policy. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. An individual who functions in a dual capacity shall meet the responsibilities and qualifications of a director set forth in §3270.34(a) and (b).
