A play kitchen in the facility's pre-K room was observed to be off balance and easily tottered. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The off-balance play kitchen in the pre-k 1 classroom has been removed from the classroom till proper repairs can be made by our facilities manager to ensure its stable. The file for child #1 was observed to lack documentation that the emergency contact and the financial agreement were updated at least once in a 6-month period. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. The file for child #2, a young toddler, was observed to lack updated health reports obtained at least every 6 months for an infant or young toddler. This was evidenced by health reports dated 08/07/2025 and 04/13/2026 a period of 8 months. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record shall contain initial and subsequent health reports. Correction: Child #2 has an updated health assessment dated 4/13/26 within her file. Inspection: Renewal. The facility admitted to entering into an agreement with a parent that required prior notice and limited access, throughout the center whenever their child was in care. The facility admitted to subsequently revoking their permission for access all together. The facility lacked a court order from a competent jurisdiction limiting the parental right of access to the child on file at the facility Requirement: A parent of a child in care shall be permitted free access, without prior notice, throughout the center whenever children are in care, unless a court of competent jurisdiction has limited the parental right of access to the child and a copy of the order is on file at the facility. Opportunity shall be provided for parents to participate in the facility's program. The operator shall maintain a yearly file which documents general announcements to promote parent participation. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. The facility was observed to have a clear spray bottle that contained a clear liquid but failed to be labeled with the bottle's contents. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The clear spray bottle containing a clear liquid has been removed from the classroom and all bottles in the classrooms have been properly labeled with their contents. Inspection: Renewal. The file for staff person #1 and #3 were observed to lack documentation of obtaining an annual minimum of 12 clock hours of child care training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: Staff persons #1 and #3 now have 12 hours of annual trainings in childcare and their certificates are housed in their staff training files. Inspection: Renewal.
