   Based on staff interviews and self-reported noncompliance from the facility, on 8/13/25 at approximately 10:15AM, Staff #1 was supervising 10 preschoolers in the downstairs back Preschool classroom. Child #1 exited the building and crossed the street to a neighbor's porch. Staff #2 observed the incident from the upstairs Infant classroom and notified Staff #1 that Child #1 had eloped. The support staff person who was present in the room for Child #1 retrieved Child #1 from the porch and returned them to the facility. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
