The most recent health reports in the file for Child #1, a preschool child, exceeded 12 months between reports. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: The child had a current physical in their file at the time of inspection it was over due by 4 days. So this was corrected before the inspection. It was due by Last day in February,(28) I did not receive it till March 4th. Inspection: Renewal. Building Surfaces (3270.76)

Observed frayed wood and an exposed nail at the edge of the porch in the outdoor play space. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Contaminated trash in closed plastic-lined receptacle (3270.67(d))

Observed soiled tissues in the trash can, which was not lidded, in the library. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Health and Safety Training (3270.31(f))

The Pediatric First Aid/CPR Certificate in the file for Staff Person #1 was acquired more than 90 days after the staff person's date of hire. The Health and Safety Trainings in the files for Staff Person #2 and #3 were conducted more than 90 days after the staff persons' dates of hire (See LIS Code Sheet). Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: Now and in the future, I will go by the HIRE date instead of the START date when counting out for 90 days Inspection: Renewal. Fire safety - 1 yr./Exp, educ., training at facility (3270.31(e)(4)(ii)/3270.192(2)(iii))

The files for staff #1, #2, and #3 lacked documentation of completing fire safety training annually. The training documentation on file for each was dated 9/2023 and with renewal dated 11/2024. Current training documentation is now on file. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: The issue was corrected before the inspection, all staff has the required trainings, The roster for verification is lost. Inspection: Renewal. Clean, age appropriate (3270.106(a))

At approximately 1:55PM, child #3 (a preschooler) was observed sleeping on a bean bag in the preschool room. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. No peeling paint or plaster (3270.77(a))

Peeling paint was observed on a floor vent near the front door in the toddler room. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: The issue was corrected with duct tape the day of inspection and the following day we replaced th with a new one. Inspection: Renewal. Building Surfaces (3270.76)

Stained ceiling tiles were observed in the following areas; one in the restroom off the large toddler room, 2 near the windows in the younger infant room, 1 in the transitional room, 1 in the small toddler room, and 4 in the break room. Additionally, two celling tiles were missing in the break room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Ceiling tiles in question have been replaced or painted over with the mildew resistant paint for the tiles. Inspection: Renewal.
