Two stained ceiling tiles were observed in two restrooms; in the restroom connected to the yellow room and the girl's restroom in the lower hall. A displaced ceiling tile with gaps was observed in the red room. Several areas of sharp metal were observed on the corrugated aluminum siding of two counters in the school age room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The stained ceiling tiles in the two restrooms were replaced and the displaced ceiling tile with gaps was placed correctly in the red room.
