On 6/30/26, Staff #1 file did not contain evidence of annual training in the facility's emergency plan. The last emergency plan training was dated 6/19/25. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: Director trained Staff #1 in the facility's emergency plan on 6/30/26. Documentation of the training was placed in the file. Inspection: Renewal. Diagnosis and treatment for emergency/Written instructions (3270.131(d)(6)/3270.133(2))

On 6/30/26, at time of renewal, an EpiPen belonging to Child #1 was observed at the facility. An allergy medical plan was not on file for Child #1. Requirement: A health report shall include a statement of the child's medical information pertinent to diagnosis and treatment in case of emergency. A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. Correction: Child #1 Allergy Medical Plan will be updated by the child's pediatrician on 7/1/26 and submitted to our office. Inspection: Renewal. On 6/30/26, at time of renewal, an EpiPens belonging to Child #1 and Child #2 were observed at the facility. Child #1 EpiPen prescription did not contain Child #1 name. Additionally, Child #1 EpiPen contained an expiration date of 5/31/26. Child #2 EpiPen contained an expiration date of 12/31/25. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label. Correction: Child #1 EpiPen will be updated by the doctor with their name on the prescription. Expired EpiPens will be replaced with new EpiPen. Inspection: Renewal. On 1/17/25, Staff #1 file did not contain results of Child Abuse and FBI clearances required by CPSL. Staff #2 file did not contain results of PA Criminal clearance as required by CPSL. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #1 may not work in a child care position at the facility. Correction: Staff #1 will be removed from the facility until results of Child Abuse and FBI clearances are obtained. Staff #2 PA Criminal clearance was previously submitted and "under review." The completed clearance was accessed today on the PA State Police site and was placed in the file. The Dissemination date is 8/28/24. Staff #1 clearances were obtained 1/21/25. On 1/17/25, Staff #3 was observed working as a staff person in the young toddler room. Staff #3 file did not contain verification of education required to qualify them as a staff person. Requirement: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. Correction: Staff #3 high school diploma was missing from the file. Staff #3 high school diploma was obtained and photocopied. It was placed in their file at 3pm on 1/17/25. Locked or inaccessible (3270.66(a))

On 1/17/25, two cans of shaving cream instructing to "keep out of reach of children" was observed accessible to children on a shelf in Room 202 (PK). Diaper cream instructing to "keep out of reach of children" was observed accessible to children in unlocked cabinets under the diapering stations located in the infant room, Room 109 (young toddlers) and Room 107 (Older Toddlers). Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Shaving cream was removed from the shelf and placed in an area inaccessible to children. The diaper stations all contained safety locks that were not engaged during the inspection. On 1/17/25, a fire drill was not conducted at the facility at least once in a 60 day period. The last fire drill on file was dated 11/7/24. Additionally, the fire alarm was not tested at least once in a 30 day period as required by Panic & Fire Act 62. The last fire alarm test was dated 11/7/24. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Correction: A fire drill and fire alarm test was conducted at the facility on 1/17/25. The fire drill and fire alarm test was recorded and will be kept on file. General Health and Safety (3270.21)

On 10/14/24, Staff #1 self-reported that two of Child #1 fingers were burned after touching baked beans served for lunch at a temperature that was too hot. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: Facility implemented an immediate action plan that includes replacing the tins with serving bowls and not placing food inside the warmer, but rather on top of the warmer in the kitchen. The warmer will be set to 50% of the original holding temp. Once ready for delivery, the meal will be placed on the serving cart at least 20 minutes before delivering to classrooms. Cook will check the internal temperature of all food before being wheeled to ensure under 110 degrees. Classroom teachers will stir all food and use glove to test temperature before plating it.
