During the Renewal Inspection on 4/8/2026, Children #1, #2 and #3 each had current health reports with the immunization history in accordance with the recommendations of the ACIP on file. However, these initial health reports with the immunization history attached were not received within 60 days following the child's first day of attendance at the facility. Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Correction: We reminded all parents that the initial report and immunization report need to be on file within 60 days of when the child starts attending. On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility. The child sustained second degree burns on the palms and hands. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Correction: Replaced heaters with Central AC + heating in playroom and sleep room. All hot water pipes have a protective guard placed over. Inspection: Complaints- Legal Location. On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility. Staff #1 applied Trombosil cream on the child's hands. During an unannounced inspection on 3/24/2026, it was discovered that the Trombosil cream was not labeled with the child's name and the parent did not provide written consent for administration of any medication. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. A parent shall provide written consent for administration of medication or a special diet. Portable space heaters/Manufacturer's instructions (3280.92(a)/3280.92(c))

During an unannounced inspection on 3/24/2026, it was observed that the facility was using a small portable space heater in both childcare spaces. The manufacturer's instructions for use for these two small portable space heaters were not in an accessible area in the facility or on file anywhere in the facility on 3/24/2026. Requirement: Portable space heaters, if allowed by local ordinance, may be used while children are in care, if the units are used in accordance with the manufacturer's operating instructions. The manufacturer's instructions for use shall be kept in an accessible area in the facility. Mixed age levels (3280.52(c))

Upon arrival to an unannounced inspection on 3/24/2026, Staff #2 was observed working alone in the Main Room with a mixed-age group of 1 infant, 1 young toddler and 3 preschool children. Two staff persons are required to maintain the required staff-to child ratios for this mixed-age group of children. After a few minutes, Staff #4 returned to the Main Room and had one child with them. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b). TIERED LIS: 1. The required staff-to-child ratios must be maintained at all times. This portion of the plan shall have an immediate correction date. 2. The operator must print and post the required staff-to-child ratios in each measured childcare space. The operator shall provide a date for when this will be completed. Correction: 1. There was an incident at the facility on 3/19/2026 where a child was injured. It was verified that Staff #1 was the only staff person present at the facility when the incident occurred on 3/19/2026. During the file review on 3/24/2026, Staff #1 had a high school diploma on file, but did not have verification of 2 years of experience working with children on file to verify that they were qualified as a primary staff person. See LIS code sheet. Requirement: A primary staff person shall have attained a high school diploma or a general educational development certificate and 2 years of experience with children. A primary staff person shall be present in a facility when six or fewer children are in care, unless the staff-child ratio specified in §3280.52(b) or (c) requires a second or third staff person. A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff person #2 may not work in a childcare position with direct contact and routine interaction with children until completed National Sex Offender Registry (NSOR) clearance is on file. On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility while Staff #1 was vacuuming the area rug in the childcare space. Staff #1 is no longer employed at the facility. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. During the Renewal Inspection on 7/2/2025, the playpens and rest mats used by the children in the facility for rest equipment were not labeled were not labeled for the use of a specific child to be used only by the specified child. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: The cribs were immediately labeled with the name of the child to be used only by that child. The rest mats were labeled with a number to match corresponding chart that was posted with the number and name of the child to be used only by that child. Inspection: Renewal. Locked or inaccessible - in original container (3280.66(a))

During the opening inspection on 4/1/2025, several cleaning materials that were labeled keep out of children were seen in unlocked cabinets below the sinks in the main childcare room and in the bathroom. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Correction: The cleaning materials were immediately removed from the unlocked cabinets and moved to an area that will be inaccessible to children. Inspection: Initial review. Refrigerator (3280.106)

During the opening inspection on 4/1/2025, the refrigerator available for the facility did not have a thermometer in the refrigerator. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below.
