In the computer room, the changing table lacked a plastic-lined, hands-free covered can next to the table to immediately discard soiled diapers. The plastic-lined, hands-free covered can was located approximately 6 feet away from the table, and staff stated that this is the location of the changing table and hands-free covered can when diaper changing occurs. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Building Surfaces (3280.76)

In the main room, there was a mirror mounted on the wall near the couch that had sharp corners that were accessible to children. On the outdoor play space on the porch, the wood lattice on the railing around the porch was broken and coming apart from the railing in multiple areas with sharp edges and had a few exposed nails with sharp edges that were accessible to children. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. In the main room, there was a power strip on the floor near the television that lacked 8 outlet covers and was accessible to children 5 years of age and younger.
