The file for staff person #1 was observed to lack documentation staff person obtained an annual minimum of 12 clock hours of child care training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include records of training required by the Department. Correction: I had more than enough credits for 2026, so I loaned 2025 2 hours from 2026. I have completed 24 hours of professional development in 2025-2026. Inspection: Renewal. Observed on the day of the inspection, the most recent fire alarm testing was conducted on 10/2/25, which was more than 30 days prior to the date of the inspection. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Observed the operator did not give the parent of child #1 the original incident report on the day an incident occurred with child #1. Requirement: A child's record must contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: Paper was signed that afternoon by mom and the original given to her and a copy placed in the child file and the facility file. Inspection: Unannounced Monitoring. Observed the facility did not have a gate secure enough to keep a family pet outside of the child care space while the operator was caring for the children. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Protective Outlet Covers 5 yrs. or less (3290.63)

Observed 2 electrical outlets which did not have protective receptacle covers, one was in the carpeted area of the child care space, and the other was where the decorative lights were plugged in. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Emergency plan (3290.24(f))

The operator was observed to be providing a letter to each enrolled child's parent explaining the emergency procedures, however, this letter failed to provide information regarding accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures described in subsection (a). The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Name/address/phone release person (3290.124(b)(7))

The file for child #1 was observed to contain an emergency contact form that lacked the address of the individual designated by the parent to whom the child may be released. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.
