On 3-19-2026 during a Renewal inspection, the mirror in the Infant room was dirty. The baby dolls, toy carrot and dish in Room 4 was dirty. The wooden toy kitchen, toy trucks and cube in Room 5 were dirty and the red bin in the wooden kitchen sink was broken. The toy castle, toy juice bottles, toy egg and boat in Room 6 were dirty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Mirror in infant room was cleaned. Baby dolls, toy carrot and dish in room 4 were thrown away. On 3-19-2026 during a Renewal inspection, a ceiling tile in the infant room was stained. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. The purple and yellow driving toy, plastic large Lego blocks and black sink bowl in the wooden toy kitchen in the Older Toddler room were dirty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Pink and blue fisher price toy and fisher price farm house were thrown away. New fisher price farmhouse was ordered. On 4-2-2025 during a Renewal inspection, the library area in the Older Toddler room had damaged plaster. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Plaster was fixed and wall was painted. On 4-2-2025 during a Renewal inspection, the first-aid kit in the Preschool room was accessible to children by hanging on the doorknob. Requirement: A first-aid kit must be inaccessible to children. On 4-2-2025 during a Renewal inspection, staff person #2 did not have a file containing the required clearances for staff persons. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. On 4-2-2025 during a Renewal inspection, the file of staff person #1 did not contain a Pediatric first-aid/CPR training certificate. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.
