Facility does not have an emergency agreement or fee agreement on file for child # 3 at the time of their enrollment on 11/11/24. The only emergency contact form and fee agreement on file is from 3/11/25, four months after child # 3 started attending the facility. Requirement: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.
