During the renewal inspection on 5/19/26, the operator did not have the emergency plan posted in a conspicuous location for the parents to view. Requirement: The operator shall post the emergency plan in the facility at a conspicuous location. Correction: The Emergency Plan was promptly posted on the facility bulletin/message board in plain view and easy access during time of inspection on 5/19/26. Inspection: Renewal. Building Surfaces (3280.76)

During the renewal inspection on 5/19/26, on the first floor near the area where the children nap, there was a window blind, a rolled carpet, and veneer flooring in the childcare space that posed as a tipping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. FIRE DETECTION (3280.95(a))

During the renewal inspection on 5/19/26, the operator did not have an attestation form or proof and date of purchase of an interconnected fire alarm system with the fire drill logs. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Correction: The missing Attestation Statement for Fire Safety Requirements for Facilities with Interconnected Systems has been completed and emailed to my inspector, Mr Fred Jones Inspection: Renewal. Pertinent Laws & Regulations/General Health and Safety (3280.14/3280.20)

During the renewal inspection on 5/6/25, the provider did not have written policy on file to address: 1. Recognition of potential sign/symptoms of shaken baby syndrome and abusive head trauma. 2. Strategies for coping with a crying, fussing, or distraught child. 3. The prevention and identification of child maltreatment. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: In accordance with Announcement C 22-03 Shaken Baby Syndrome, Abusive Head Trauma and Child Maltreatment a policy was developed and implemented in the facility. It has been poste predominately where it can be seen by all. The policy was also distributed to the parents and a signed copy added to the child's file. Menus (3280.165)

During the renewal inspection on 5/6/25, the provider did not have verification that she discussed the general menu plan with the parents. Requirement: The operator shall discuss the general menu plan with the parent so that the daily nutritional needs of the child can be met. FIRE DETECTION (3280.95(a))

During the renewal inspection on 5/6/25, the provider did not have documentation of fire detection device testing for the month of April 2025. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Correction: In accordance with section 1016c of the act (62 P.S. § 1016(c) the fire detection device was tested and annotated. Inspection: Renewal.
