The floor in the hallway from the front door to the kitchen was observed to have area rugs over the floor. Under the rugs, the floor has been removed and boards where the floor should be was seen. The baseboard in the hallway from the front door to the kitchen was removed and not replaced, exposing parts of an unpainted unfinished wall. The T-strips that joins the floor in the family room and the flooring the kitchen, had a broken piece located the right corner of the kitchen an\d family room. The ceiling in the family room was observed to have water marks and there was a 2 inch hole in the ceiling. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Correction: T strip that joins the floor in the family room and kitchen will be repaired. All floors' walls and ceilings will be repaired. It was observed that Staff Person #1, who was present in the back living room, could not properly supervise the children in care who were playing in the front room area. Staff person #1 could not see or redirect the children who were observed to be climbing on top of the furniture and jumping off, creating unsafe conditions at the facility. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3290.51-3290.52 (relating to maximum number of children; and staff:child ratio). Correction: A piece of carpet was added to the doorway leading into the playroom to cover the damaged carpet and tac strip.
