The file for staff person #1 included a FBI Clearance which was not updated within 60 months of the previous clearance. The current FBI Clearance for staff person #1 is dated 9/22/25 and the previous FBI Clearance was dated 9/2/20. The file for staff person #2 included a NSOR Certificate which was not updated within 60 months of the previous clearance. The current NSOR Certificate for staff person #2 is dated 3/20/25 and the previous NSOR Certificate was dated 3/13/20. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Staff will be given a 30-day reminder that their clearances are due. They must be within 60 months of the date on their previous clearances. If the staff person's clearance has not been returned, the staff person will be removed from direct supervision of children (classroom) and assigned other center tasks until the clearance is returned. Documentation will be placed in the staff file, stating the date they were removed from the classroom and that they were not directly supervising children. It was observed in the toddler classroom a bottle of Isopropyl Alcohol was being stored in a cabinet under the handwashing sink that was not locked or made inaccessible to children. It was observed in the toddler classroom, a basket of diaper creams being stored under the changing table accessible to children due to not have a working child safety lock. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: A child lock was placed on the lower cabinet to prevent children from accessing it, and Alcohol was removed. A child lock was installed on the lower cabinets to prevent access beyond the stools in front of the cabinets. The broken child lock on the changing table was removed and replaced. It was observed in the young toddler classroom, a spray bottle containing a blue cleaning solution was not in its original labeled container or in a container that specifies the content. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: "Soapy Water" will be added to the indicated spray bottle, and/or any other bottles will be properly labeled with the specific names of the other substances contained. It was observed in the toddler classroom that the first aid kit was missing tape and tweezers. (This was corrected on site) Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: Tape and tweezers were added to the toddler room first aid kit. First aid bags were checked, and used items were replaced as needed. It was observed in the toddler classroom the top drawer at the meal prep area was unlocked and contained sharp knives and adult scissors. It was observed in the bathroom used by toddlers, a power cord was unplugged and hanging from the paper towel dispenser. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Items were removed from the top drawer of the island and placed in another location still out of reach of children, but accessible to staff for meals. The power cord was removed, and the cover was placed over the outlet. Ziplock bags were removed from the island and stored out of reach of children, but still accessible to staff. It was observed during inspection in the young toddler classroom a buildup of dust, dirt and grime in the corners of the classroom, at the baseboards, behind the toy shelves and at the sink area where children was their hands. It was also observed during inspection in the preschool classroom a buildup of dust, dirt and grime in the corners of the classroom, at the baseboards, behind the toy shelves and in the bathroom within the classroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: ELC is working with the facilities manager to have all areas deep cleaned, beyond the daily cleaning provided by the cleaning service. Classroom floors are being placed on a deep clean and waxing schedule to better maintain and preserve the floor surfaces. Inspection: Complaints- Legal Location. It was observed in the toddler classroom, a bottom drawer of the meal prep area contained plastic Ziplock bags filled with plasticware accessible to toddlers who are still placing objects in their mouths. It was observed in the toddler classroom, plastic Ziplock bags filled with toys and sensory items with a diameter of less than 1 inch, on a bottom shelf behind the changing table accessible to toddlers who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Teachers removed toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags, and styrofoam objects, as these may not be accessible to children who are still putting objects in their mouths. Ziplock bags that were filled with cutlery were removed, and new storage containers have been used. All sensory items contained in the Ziplock bags on the shelf behind the changing table have been removed and stored elsewhere. Staff assigned to specific children (3270.113(a)(1))

It was observed in the young toddler classroom that children laying on their nap cots were not able to be properly supervised by staff. The nap cots were placed in areas that caused children to be out of sight from staff. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff will always place themselves and nap cots in a location were they can see, hear and assess all children during nap time. They will also always see, hear, and assess children while assisting other children if they happen to stir during naptime. Position themselves in one location and can move throughout the classroom.
