A renewal inspection was conducted on 4/9/2026. At that time, an unlabeled bottle containing a cleaning solution was observed in the Pre-K room. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: Unlabeled bottle containing bleach water was labeled with name of contained cleaning solution while inspector was present. Inspection: Renewal. A renewal inspection was conducted on 4/9/2026. At that time, the fire detection testing log was reviewed. Smoke alarms were tested past 30-day requirement on the following occasions: 3/11/2025-4/11/2025, 5/7/2025-6/7/2025, 6/7/2025-7/10/2025, 7/10/2025-8/10/2025, 8/10/2025-9/10/2025, and 10/10/2025-11/10/2025. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. A renewal inspection was conducted on 4/9/2026. At that time, the facility's emergency plan letter was reviewed. Emergency plan letter given to parents at enrollment did not provide for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. Requirement: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. A renewal inspection was conducted on 4/9/2026. At that time, a sample of children's files was reviewed. The emergency contact form for Child #3 was missing signed parental consent for emergency medical care and minor first-aid. Requirement: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Transportation First Aid Kit (3270.178)

A renewal inspection was conducted on 4/9/2026. At that time, there was no first-aid kit in the Honda Pilot which is used for transportation. Additionally, there was no soap in the first-aid kit in the van. Requirement: A first-aid kit, including the contents as specified in §3270.75 (relating to first-aid kit) shall be in the vehicle when children are being transported. A renewal inspection was conducted on 4/9/2026. At that time, staff files were reviewed. Most recent health assessment on file for Staff Person #1 is dated 3/13/2024, which is more than 24 months ago. Initial health assessment on file for Staff Person #4 is dated 6/7/2024, which is more than 12 months prior to providing initial service in a child care setting (see LIS code sheet for start date). Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. A renewal inspection was conducted on 4/9/2026. At that time, a sample of children's files was reviewed. The following information was missing from children's emergency contact forms: Child #3: Health insurance coverage and policy number. Child #4: Address of child's physician. Child #5: Health insurance policy number. Requirement: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care. Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. A renewal inspection was conducted on 4/9/2026. At that time, there was no thermometer in the refrigerator in the kitchen. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: Thermometer missing from kitchen refrigerator was replaced while inspector was present. Inspection: Renewal. A renewal inspection was conducted on 4/9/2026. At that time, staff files were reviewed. The following staff files did not contain necessary items: Staff Person #2: Most recent DHS FBI clearance is dated 4/8/2021, which is over 60 months. Staff Person #3: Most recent Child Abuse clearance on file is dated 1/21/2020, which is over 60 months. Staff Person #4: Does not have DHS FBI clearance on file. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #2, Staff Person #3, and Staff Person #4 may not work in a child care position with direct contact and routine interaction with children until all completed clearances are on file at the facility.
