    1. Personnel (SERIOUS) - A personnel kicked a child causing the child to fall down. Complaint finding: Substantiated (complaint #20127). Plan to correct: During investigation, the employee was terminated. Administration documented the incident and cooperated fully with Licensing. All staff will receive retraining on Oklahoma DHS discipline and behavior guidance policies, specifically prohibited physical discipline and appropriate redirection techniques per OAC 340:110-3-288. Staff will sign documentation acknowledging understanding of these policies and expectations.
