Two staff members who have been employed more than 12 months have an expired PDL. Visit: Periodic / Full. Plan to correct: One staff member has already applied for her renewal and one staff member will reapply today. Program director will continue to remind staff about their PDL, and will also print a registry report and highlight who is about to be expired to hand out to the staff who are about to expire. Continuing professional development

Two staff members employed more than 12 months did not complete the required number of professional development hours for their PDL renewal. Visit: Periodic / Full. Plan to correct: CCLS is sending a free training course that both staff members can complete to give them enough training hours for their PDL renewal. One staff member employed more than 12 months has an expired PDL. Visit: Periodic / Full. Plan to correct: The staff member applied for this PDL on 4/21/25, but did not have enough training hours. She has enough training hours to renew this PDL now and has reapplied on 6/25/25. The staff member will send in her CPR card front and back for her PDL hours. Director will ensure that staff members are checking their emails from CECPD to follow-up after they submit PDL renewals to ensure the PDL's are renewed. CPR and first aid

One staff member employed more than 90 days does not have an approved CPR/1st Aid course. Visit: Periodic / Full. Plan to correct: Staff member will take a new CPR course through an approved source. Director will check with Cherokee Nation Resource and Referral for an in-person training. If they don't have a course, she will contact EMS. Food-contact items and surfaces

Plastic food utensils are stored in a box with an open lid under a food prep table in the kitchen. Visit: Follow Up / Partial. Procedures

Mice droppings found throughout the kitchen on surfaces and on different food items such as cans, boxes with baby food containers, and box with mayo containers. Visit: Follow Up / Partial. Plan to correct: Program is going to clean/sanitize everything in the kitchen area. Executive Director held a meeting with Food Program coordinator and they are going to have a daily checklist that they will have kitchen staff use for the kitchen cleaning. Pest control

The program had a health inspection on 12/11/24, and mice droppings were found. CCLS observed mouse droppings in the kitchen area of the program. Visit: Follow Up / Partial. Plan to correct: Program is going to clean/sanitize everything in the kitchen area. The program had an exterminator come on November 1st and November 27th, and the exterminator is scheduled to come back on December 17th. Program will send the exterminator summary to CCLS.
