Type and quantity

Equipment in Tod 2B, Tod A, Pre B, and PreK classrooms does not meet the minimum amount of sets required on the Equipment Inventory for amount of children in classroom. Facility

Electrical covers were missing in the 3 year old rooms. Middle toilet in preschool room leaking. Visit: Periodic / Full. Weekly lesson plans

Diaper changing area

Parent permission

Diaper cream and prescription medication found in Pre K classroom without current authorization. Visit: Complaint / Full. Additional Non-Compliance Found During Investigation: Caring for and Educating Children- Child's labeled formula bottle with Metamucil in it was given to wrong child. Staff confirmed this incident Complaint finding: Substantiated (complaint #18162). Plan to correct: Family that reported this did not notifty the center that there was Metamucil in their child's bottle.
