Labeling and storing

5 diaper creams in Infant B was not labeled with the child's full name. Visit: Periodic / Full. Plan to correct: Creams have been pulled from the classrooms. Children were playing on the boards surrounding the mulch on the preschool playground. The boards are loose and broken. Visit: Periodic / Full. Inappropriate Discipline (SERIOUS): A child was grabbed, forced to move, and slammed which resulted in the child bumping their head on the wall. Complaint finding: Substantiated (complaint #19535). Plan to correct: We retrained all teachers on how to help children with discipline issues. Licensing observed standing water in outdoor play ground in a water table. Visit: Periodic / Full. Plan to correct: Licensing typed for director: Director immediately dumped the standing water out of the water play equipment.
