Vehicles and vehicle maintenance

Licensing was unable to view a current quarterly inspection for all three buses. Visit: Periodic / Full. Plan to correct: Director reached out to transportation manager during the visit and transportation manager advised they will send the quarterly inspection to Director after they are done transporting for the afternoon. Oklahoma Professional Development Ladder

One employee, employed longer than 12 months, has an expired PDL. Visit: Periodic / Full. Plan to correct: Director will have employee check on her certificates and send to CECPD and Licensing. Fencing

Portions of the fence, even with the tension wire, allow enough space for children to crawl under, due to erosion of dirt. Visit: Periodic / Full. Plan to correct: Director will contact the school and notify the Superintendent it needs to be fixed. During the visit, Director informed her supervisor and Health & Safety Manger of the concern. Plan to correct: During the visit the Health & Safety Manager contacted maintenance for the program. The Health & Safety Manager was advised maintenance were going to install the mulch today. Vehicle insurance

Current auto insurance policy not on site. Visit: Periodic / Full. Plan to correct: Program Director contacted the GCBH Health & Safety Coordinator during the visit who agreed to email the current insurance policy to Program Director as soon as the bus driver finished their route. Program Director will email Licensing the current policy. Program Director will set herself a reminder to request the new auto insurance policy before the expiration date. Vehicle maintenance

Current quarterly vehicle inspection logs not on-site. Visit: Periodic / Full. Plan to correct: Program Director contacted the GCBH Health & Safety Coordinator during the visit who agreed to email the most recent vehicle quarterly inspection logs to Program Director as soon as the bus driver finished their route. Program Director will email Licensing the maintenance logs. Program Director will find out when the buses are inspected & set due dates to request the log. Fencing

The fence on the Head Start Playground does not begin at ground level & there is enough space for children to fit under. Visit: Periodic / Full. Plan to correct: **Although this continues to be a non-compliance, the Plan of Correction (POC) date has not expired.** Program Director will email the Health & Safety Manager for GCBH, who will then contact school officials regarding the fence. Staff will inform Program Director if there are any safety issues with the outdoor play area. Impact material under required equipment is less than 6 inches. Visit: Periodic / Full. Plan to correct: **Although this continues to be a non-compliance, the Plan of Correction (POC) date has not expired.** The impact material was delivered on 11/04/24 but has not been installed due to the weather. The impact material will be installed by the due date of 11/28/24. Staff will inform Program Director if there are any issues with the impact material.
