Licensing observed central detection system last tagged and inspected by Guardian Security Systems on 3/21/2025. Visit: Periodic / Full. Plan to correct: Director will call quality assurance manager today and inform that the central detection systems needs renewed. Director will request that an appointment be set for inspection by the end of the week. Director will email Licensing when appointment is scheduled and a copy of the tag after inspection. Supervision
*SERIOUS* 3/7/2025 Self Report: Supervision - A 4yr old child left the classroom and walked out the front door into the parking lot where his mom saw him and brought him back into the classroom. Center Director that normally watches the front door welcoming parents, but on that day she was covering as the EHS teacher. The child was found missing when the substitute cook had gone to the other classroom asking for the child count for breakfast and about any food allergies at which time the lead teacher turned around to inform the substitute cook. Visit: Periodic / Full. Plan to correct: Terminate lead teacher 2. Hire new qualified teacher 3. Provide additional intense Active Supervision training to all staff 4. Re-modeling the classroom to ensure no blind spots. HS will be closed March 12-March 14 for construction to take place. HS will also be closed for Spring Break and finalization for the classroom will take place. In the future, supervision will be enforced at all times. Source: OKDHS Child Care Locator program record.
