Parent notifications

Out of a sample of child files reviewed, 4 of 6 had expired compliance file notification forms. Visit: Periodic / Full. Plan to correct: Staff in charge is going to contact all parents and have them complete new compliance file notifications so that all compliance file notifications will be due at the same time. In the future all compliance file notifications will be completed by all parents during the same week. Parent notifications

One child record did not include a current compliance file notification. Visit: Periodic / Full. Plan to correct: Program director is going to print a new compliance file notification form and request the parent sign it next time the child is in attendance. Before a child's first day at the program, Program Director will make sure all children have a current compliance file notification in their child record. Oklahoma Professional Development Ladder

One staff member employed over 12 months had an expired PDL. Visit: Periodic / Full. Plan to correct: Program Director is having the employee apply for her PDL today. Staff member already has enough hours to qualify. Program director will keep in contact with the programs education coordinator and watch out for PDL's that will expire soon.
