    Licensing observed an infant older than 3 months swaddled. Visit: Periodic / Full. Plan to correct: Licensing observed a teacher remove the infants arms from the swaddle/sleep sack, allowing the child to now sleep in a sleep sack with their arms free. Director will require safe sleep training again for all staff and will monitor rooms throughout the day for infants sleeping in safe sleep environments and not to be swaddled. Director stated she will do an investigation into which teacher is responsible and they will receive a disciplinary action along w/reviewing the sleep sacks to determine if they want to keep them at their program since she was unaware the sleep sacks had the ability to restrict arms as well.
