Appropriate sleep environment

Licensing observed an infant older than 3 months swaddled. Visit: Periodic / Full. Plan to correct: Licensing observed a teacher remove the infants arms from the swaddle/sleep sack, allowing the child to now sleep in a sleep sack with their arms free. Director will require safe sleep training again for all staff and will monitor rooms throughout the day for infants sleeping in safe sleep environments and not to be swaddled. Director stated she will do an investigation into which teacher is responsible and they will receive a disciplinary action along w/reviewing the sleep sacks to determine if they want to keep them at their program since she was unaware the sleep sacks had the ability to restrict arms as well. Plan to correct: Licensing observed a teacher remove the infant from the rug on the floor and place the infant on their back in a crib. Director will require safe sleep training again for all staff and will monitor rooms throughout the day for infants sleeping in safe sleep environments. Position specific professional development

One staff employed over 90 days does not have current CPR-First Aid from an approved source. Visit: Periodic / Full. Plan to correct: Asst. Director enrolled the staff member in the next CPR-First Aid class (2/19/26) during the visit. Asst. Director will email CCS Supervisor the new card. In the future, Asst. Fire extinguishers and automatic sprinkler systems

The fire extinguisher was last inspected 11/2023. Visit: Periodic / Full. Plan to correct: Program is going to contact Western Fire State Protection services to come back to the program and inspect the fire extinguishers. Program Director will make sure the company inspects all extinguishers when they come to complete the inspection report. Physical environment checklist was last complete 12/2024. Visit: Periodic / Full. Plan to correct: Program will complete new physical environment checklist for the program. Program is going to complete the physical environment checklist now, and then they are planning to complete this again in December 2025 to make sure they are completed before they are due. One staff member employed more than 12 months has an expired PDL. Visit: Periodic / Full. Plan to correct: Program applied for the staff members PDL while CCLS was on site. Program just made a new checklist of all staff members, so program is going to attach a copy of it with the licensing reports that she prints off of CECPD to make sure she doesn't miss anyone's PDLs.
