Licensing received 2 personnel information forms by email on 4/2/26 and both personnel were hired in February 2026. Licensing viewed personnel were uploaded to CCMASS file cabinet on 4/1/26. Labeling and storing

Licensing observed Destin diaper cream in the infant room, one's room, and a two-year-old's classroom that did not have the child's full name. Visit: Periodic / Full. Plan to correct: All diaper cream bottles will labeled. All child will have their own each individual container with their first and last name on the bottle. Diaper cream will not be shared. Required individuals

Licensing observed a personnel, who was a previous employee with the last date of employment as 6/27/25, working in a classroom that did not have a criminal history review request resubmitted to OBI for new employment date. Personnel new employment date is 9/24/25. Visit: Periodic / Full. Plan to correct: Licensing had personnel submit background request to OBI. Request ID#979101. Before a new personnel's start date the program will check and make sure the individual background has been submitted to OBI. In the future will maintain requirements. Oklahoma Professional Development Ladder

Licensing observed one personnel (LO) with a PDL that expired on 5/23/25 and PDL last submitted on 5/23/24. Visit: Periodic / Full. Plan to correct: Director had personnel to submit PDL immediately. Director will talk with personnel to reinstate to submit PDL's after completion of training and prior to 60 days. In the future will maintain licensing requirement. Oklahoma Professional Development Ladder

Licensing observed 3 personnel with expired PDL's. The 3 personnel submitted their PDL application for renewal at least 26 days or more. However, training hours on personnel AP show 0, AD completed on 1/3/25, and IW completed on 1/4/25. Visit: Periodic / Full. Plan to correct: 2 personnel currently show 20 hours of training. 1 personnel shows zero training. Director showed licensing certificate for completion for other personnel for 20 hours of training. The director will have personnel have training hours completed 60 days prior to PDL expiration and personnel will submit PDL application for renewal no later than 60 days.
