Plan to correct: Playground Procedures Policy To maintain a safe and consistent environment for all children and staff, the following procedures must be followed at all times: 1. Playground Access - No class is permitted to use their classroom door to enter or exit the playground. - All classes must use the designated hallway exit and entry points assigned for playground transitions. 2. Name-to-Face Verification • Classroom teachers will complete a name-to-face check before leaving the classroom to go outside. • Classroom teachers will complete a name-to-face check before leaving the playground to come back inside. • An administrator will verify each name-to-face check before the class goes outside and again before the class comes inside. • Classroom teachers will only complete each name-to-face check once—once before leaving the classroom and once before leaving the playground. 3. Communication Equipment - Walkie-talkies are required whenever a class is on the playground. - Walkie-talkies will be stored on the metal shelf outside the laundry room. - Staff must ensure a walkie-talkie is taken outside and remains functional during playground use. 4. Staff Responsibility - Classroom staff are responsible for ensuring all procedures are followed. - Any concerns or safety issues must be reported to administration immediately. Playground Fencing Currently the children will enter and exit the front playground through the Yellow 1 classroom, and once the fencing is extended, they will utilize the front exit. Temperature

Documented on Health Inspection Conducted on 06/25/2025 - Milk was being held in a reach in cooler that was between 50-58 degrees. Program discarded milk at the time of the inspection. Visit: Periodic / Full. Plan to correct: Program has redistributed the milk so that the individual cartons were evenly spaced to allow the fridge to maintain the proper temperature and discussed with staff the proper way to stock the milk cartons in the fridge. Children's attendance

During the OIG visit conducted on 04/03/2025 it was determined the provider did not maintain adequate attendance records, including arrival and departure times, for all children in care.
