Appropriate sleep environment

Licensing observed pacifier with attachment in crib with an infant. Visit: Complaint / Full. Plan to correct: The pacifier attachment was removed from the crib while Licensing was present in the room. Director will have all staff signed up for additional training pertaining to infants and safe sleep and have ongoing conversation with staff regarding safe sleep practices for infants. Entry Level Child Care Training (ELCCT) or equivalent

Four staff with hire dates of 6/30/2025, 7/21/2025, 1/24/2025, and 6/12/2025 did not have ELCCT training. Visit: Periodic / Full. Plan to correct: Ms. Meli will have two staff members complete their ELCCT training today. Ms. Meli will have the other two staff members work on getting access into their CECPD accounts today. Position specific professional development

One staff CPR First Aid certification expired 7/31/2025. Five staff with hire dates of 11/18/2024, 5/21/2025, 6/30/2025, 1/24/2025, and 6/12/2025 did not have CPR First Aid certifications. Visit: Periodic / Full. Plan to correct: Ms. Meli will have all staff complete CPR First Aid training in December 2025. Inappropriate sleep environment

Licensing observed infant asleep in swing, staff stated child was asleep and in swing for at least 10 minutes. Visit: Complaint / Full. Plan to correct: Corrected during visit, staff moved infant to proper rest equipment. In the future, children under 12 months old that fall asleep in swings or other inappropriate rest equipment will be moved immediately. 340:110-3-296(b)(2)(B)(ii) — When an infant falls asleep in equipment other than appropriate rest equipment, the infant is immediately moved to appropriate rest equipment.

Rest time- Infant in an inappropriate sleep environment Complaint finding: Substantiated (complaint #19131). Plan to correct: To reinforce safe sleep practices, swings have been removed from the infant room. Individual staff meetings have already covered safe sleep guidelines. We will be reviewing these guidelines again at the staff meeting on August 13th. All staff will also complete a safe sleep refresher course beginning with those who work in the infant room and all others throughout the year.
