    Licensing observed dog rabies vaccination expired May 2026. Visit: Periodic / Full. Plan to correct: Primary Caregiver has scheduled an appointment for dog rabies vaccination. Dog was experiencing stomach issues at last veterinarian appointment and vet prescribed medication and postponed vaccination. Dog will receive rabies vaccinations by 8/8/2026. Plan to correct: Second caregiver arrived within 10 minutes of the visit to correct non-compliance making the ages and number of children six children in care with two caregivers, four of the children under 2 years of age.
